These resource maps are maintained as part of our ongoing commitment to addressing social determinants of health and improving community well-being.

The maps and county snapshots below are informed by HealthSource’s Community Needs Assessment, which identifies health trends, disparities, and unmet needs across our eight-county service area. The assessment helps ensure that our services and community partnerships are aligned with the areas of greatest need.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
