This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
HealthSource of Ohio conducts Community Health Needs Assessments (CHNA) following the Health Resources and Services Administration (HRSA) guidelines. This data and analysis serves as a baseline for HealthSource of Ohio to inform and improve upon the delivery of health services offered to patients in our communities. The CHNA analyzes county, state, and national data related to access to care, social determinants of health and health disparities, common causes of morbidity and mortality, and other unique health care needs present in our communities.
HealthSource of Ohio’s service area includes eight counties in Southwest Ohio: Adams, Brown, Clermont, Clinton, Fayette, Hamilton, Highland, and Warren.
The CHNA may be used to assess the eight-county service area as a whole, or broken down, county-by-county, to target specific populations. County-level infographics have also been created as a result of the CHNA, to better demonstrate the unique needs present in HealthSource of Ohio’s individual communities.
