Wolves Wellness School Based Health Center

HealthSource of Ohio opened the West Clermont School Based Health Center in February 2018. The services provided in this health center include primary medical care, vision services, and dentistry to any West Clermont School District student, school district staff, and the entire community. The school-based health center is open Monday through Friday and will remain open during school breaks including the summer.

Wolves Wellness Goals & Objectives:

  • Identification, referral and follow-up of health problems that may interfere with student achievement
  • Assure immunizations for protection against vaccine-preventable diseases
  • Prevention and control of communicable diseases
  • Health education directed toward student health and wellness
  • Participation in school-based research to improve the quality of school health services

Wolves Wellness School Based Health Services

  • Asthma and other chronic illness diagnosis and management
  • Acute illness and injury treatment
  • Immunizations and vaccinations
  • Routine lab tests
  • Medication prescriptions
  • Health education, counseling and wellness promotion
  • Fitness and nutrition education and counseling
  • Referrals for healthcare services not provided at the clinics, including mental health, vision and dental services, and other medical services
  • Case management for acute and chronic health problems
  • Vision screenings
  • Complete eye exams
  • On site eyewear fittings & adjustments

HealthSource West Clermont Vision Center

Children need to see clearly to learn. However, in Ohio, it is estimated that only one in seven kindergartners has an eye exam prior to entering school, while one in four school age children have a vision disorder that affects their ability to learn. Uncorrected vision problems can impair child development, interfere with learning, and even lead to permanent vision loss; early detection and treatment are critical. To help address this issue, HealthSource of Ohio and West Clermont Local Schools are teaming up together to help ensure students are receiving the vision services they need.

Transportation

Students who attend West Clermont Schools may be transported during their school by the West Clermont transportation staff. There is no additional cost for this service and transportation routes are designed to keep students in their learning environment as much as possible.

Parents will be given prior notice for all preventive appointments. They may also call to make an acute appointment during the day, or the school health aide may do so on behalf of the parent if a child is presenting signs of illness.

Van transportation will be provided from the following schools to the Wolves Wellness School Based Health Center located at West Clermont Middle School, 1341 Clough Pike, Cincinnati, OH 45103.

Middle School Students will be issued a pass and directed to the Wolves Wellness School Based Health Center located on-site.

High School Students with driving privileges will be issued a pass and permitted to drive themselves to the Wolves Wellness School Based Health Center located at West Clermont Middle School, 1341 Clough Pike, Cincinnati, OH 45103.
