About This Location

Location & Overview

HealthSource Goshen is located at the rear of Marr/Cook Elementary School in Goshen, at 6696 Goshen Road.

The center is open to the entire community and also serves the Goshen Local School District as a School-Based Health Center. HealthSource Goshen is open year-round, including during school breaks.

HealthSource Goshen is accepting new patients of all ages. We accept most insurance plans, including Medicaid, Medicare, and private insurance. A discounted sliding fee program is available for uninsured patients.

Family Practice Care

Our family practice (family medicine) clinician provides care for patients of all ages, from newborns to seniors. We offer comprehensive, ongoing care for individuals and families, including:

Sick visits and minor injuries
Annual checkups and preventive care
Care for chronic conditions such as diabetes and high cholesterol
Medication prescriptions
Ongoing primary care for the whole family

School-Based Health Services

HealthSource Goshen offers school-based health care for students in the Goshen Local School District, making it easier for families to access care close to school.
