HealthSource Goshen Family Practice and School-Based Health Center is located at the rear of Marr/Cook Elementary School. The Center is open to the entire community and also serves the Goshen Local School District as a School-Based Health Center. The Center is open year round, even during school breaks.
Our Family Practice Clinician, also known as Family Medicine Clinician, can treat patients of all ages, from newborns to seniors. They provide comprehensive and continuing health care for both males and females. Your Family Practice Clinician may treat you and your family members for acute conditions, such as a minor injury, the flu, or a sore throat. He or she may also treat patients for chronic conditions such as diabetes or high cholesterol.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
