Location & Overview
HealthSource Lebanon Family Practice is located on Columbus Avenue, in the shopping center near the new Tractor Supply store. We are accepting new patients of all ages.
We accept most insurance plans, including Medicaid, and offer a discounted sliding fee program for uninsured patients.
