HealthSource Lebanon Family Practice is located on Columbus Avenue, in the old Blockbuster store, near Kroger.

Our Family Practice Clinicians, also known as Family Medicine Clinicians, treat patients of all ages. They provide comprehensive and continuing care for both males and females. Your Family Practice Clinician may treat you and your family members for acute conditions such as a minor injury, the flu, or a sore throat. He or she may also treat patients for chronic conditions such diabetes or high cholesterol.
