Location & Overview

HealthSource West Clermont Family Practice and School-Based Health Center is located inside West Clermont Middle School.

We are accepting new patients of all ages and accept most insurance plans, including Medicaid. A discounted sliding fee program is available for uninsured patients.

Family Practice Care

Our family practice (family medicine) Clinicians offer comprehensive, ongoing care for patients of all ages, from children to adults.

Services include:

Sick visits and minor injuries
Annual checkups and preventive care
Care for chronic conditions such as diabetes or high cholesterol
Medication prescriptions
Ongoing primary care for individuals and families

School-Based Health Services

HealthSource West Clermont also serves as a School-Based Health Center for the West Clermont School District.

We can see students, teachers, and school staff for care during the school day, helping reduce missed class and work time.
