HealthSource West Clermont Family Practice and School Based Health Center is located inside West Clermont Middle School. This center offers Family Practice, Vision, and Dental services under one roof.
Our Family Practice providers treat patients of all ages. They provide comprehensive and continuing health care for both males and females. Your Family Clinician may treat you and your family members for acute conditions, such as a minor injury, the flu, or a sore throat. He or she may also treat patients for chronic conditions such as diabetes or high cholesterol.
HealthSource West Clermont can see students, teachers, and staff for treatment during the school day.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
