HealthSource West Clermont Update
HealthSource of Ohio is committed to providing you with consistent, high-quality care, and we want to share an important update regarding our services connected to the West Clermont Middle School.
Since 2018, HealthSource has partnered with West Clermont Local Schools to provide medical, dental, and vision services. As the school district continues to grow, the space currently used for our HealthSource West Clermont practice will transition back to instructional use.
Friday, May 15th will be the last day that HealthSource will see patients at the West Clermont location and will begin to see patients at the identified locations listed below on May 18th.
Beginning May 18, 2026, services will be provided at the following HealthSource locations:
Medical services Care will continue with Sam Vining, C-FNP, and her team at HealthSource Batavia
2055 Hospital Drive, Suite 130, Batavia, OH 45103
Dental services Care with Dr. Tanya Sackrider, DDS will continue at HealthSource Eastgate Dental located in the lower level of our pediatric office
4627 Aicholtz Road Cincinnati, OH, 45244
Vision services with Dr. Donna Morhous, OD will continue at HealthSource Eastgate location, located on the upper level of our pediatric office at
4627 Aicholtz Road, Cincinnati, OH 45244
These transitions will provide access to additional on-site services such as Behavioral Health, Diabetes Education, and Pharmacy services (Batavia). HealthSource is working carefully to ensure a smooth transition and to minimize any disruption to your care.
